We need some information from you in order to create a MyChart account and receive the COVID vaccination. Please read the following instructions carefully. Completing this form creates a NEW Denver Health record. Please complete this only ONE time to avoid multiple records on yourself.

Existing Denver Health patients: If you have previously been treated at DH, you already have a record. DO NOT COMPLETE THIS FORM. To log into your MyChart account, https://mychart.denverhealth.org

If you do not have access to MyChart, please schedule an appointment with the Appointment Center at 303-436-4949.

New patients: After you complete this form and your medical record is created, you will be asked to create a MyChart username and password on the next screen.
