MyChart Medical Record Access - Terms and Conditions
Acceptance of these Terms and Conditions is required to use MyChart to access my medical information online.

What is MyChart Medical Record Access? MyChart is an internet application that allows Denver Health patients to have secure web-based access to part of their Denver Health electronic medical record. It also allows a secure, convenient, communication channel with Denver Health for non-urgent messages. Use of MyChart is optional.

Certain information may not be displayed in MyChart if it is prohibited from being shared as a result of state or federal laws/regulations. However, to obtain a complete copy of my medical record, I can contact Denver Health’s Health Information Management Department.

My Responsibilities
By using MyChart, I agree to be bound by all the terms and conditions of this agreement (“Agreement”). Denver Health may modify this Agreement at any time, and I agree that modifications are effective immediately upon posting of the modified version. If I do not agree to this, I will not use MyChart.

I am solely responsible for the consequences of any sharing of my MyChart login information and medical record content that I intentionally or unintentionally communicate to others.

Password and Login Name
My login name and password are key to protecting the confidentiality of my information. I will NOT SHARE my password with anyone. It is my responsibility to keep my password confidential. It is only known by me and can be changed online at any time by me.

Secure Messaging Guidelines
I can use MyChart to send secure messages to Denver Health and my care providers. Messages that I send will be routed to the appropriate department as necessary for handling. Denver Health employees, other than my healthcare provider, may be involved in addressing my request. Therefore, I should not use MyChart messages to send very sensitive information I wish to discuss directly with my provider.

Denver Health will do its best to provide a timely response to messages. Typical turnaround time for responses is two (2) to three (3) business days.

I know that I should NOT use MyChart to send any messages requiring urgent attention. Instead, I should call the clinic directly. If it is an emergency, I will go to the nearest emergency department or call 911.

Denver Health Communications and Test Results
Denver Health may use MyChart to send me messages, test results, and other communications. Therefore, I may not receive other types of communications from Denver Health in these areas. Per provisions in the 21st Century Cures Act, certain test results and clinical notes will be uploaded to MyChart immediately, before a clinician has had the opportunity to explain them to you. You understand that there may be a lag between the time such information is uploaded to MyChart and follow-up communication with the clinician.

Minor Eligibility
An individual must be age 12 or older to request a personal MyChart account. Parents or guardians of minor children may request full MyChart access for their children between the ages of 0 - 11 years of age. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient may not view without consent of the minor patient. Because of these requirements, Denver Health cannot offer full MyChart access to parents of minor patients age 12 and above. Thus, when a minor patient reaches age 12, a parent or guardian’s access will be limited (access to medical information is disabled). Once the minor reaches age 18, the parent/guardian’s account is automatically disabled. Note that parents’ access to a minor’s records is separate from Proxy Access as discussed below.

Proxy Access
Patients may wish to give another person full or partial “proxy” access to their MyChart account. Patients may add a proxy from within their own MyChart account. I can also ask my health care provider to set this up. Denver Health may grant Proxy access to persons who have or can show a legal right to access the patient’s medical information or if the patient requests someone have proxy access. If Proxy Access is granted, a MyChart account will be activated for both the proxy and for the patient. If your relationship with your proxy changes, it is your responsibility to tell Denver Health immediately by calling Denver Health at 303-602-4380 or by sending written notice to Denver Health, Attention: MyChart, 301 W. 6th Ave., MC 0296, Denver, CO 80204. As a patient you may also remove proxy access from within your own MyChart account. Denver Health reserves the right to revoke proxy access at any time for any reason.

MyChart has links to third party web sites that are not managed or controlled by Denver Health. These web site links are for my informational purposes only. Denver Health does not endorse and has not verified the accuracy of the information in/on these web sites. I should not rely on any of the information found on the web sites for purposes of treatment or diagnosis. If I decide to access any third party sites linked from MyChart, I do so entirely at my own risk.

Security and Confidentiality
Denver Health is committed to protecting the confidentiality of my medical information. Denver Health affords the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Denver Health in any other medium. Firewalls, passwords, encryption, and audit trails are all used to safeguard my MyChart information.

Discontinuing Use of MyChart Medical Record Access
I can cancel my MyChart access at any time by contacting Denver Health at: 303-602-4380 or by asking a staff member at Denver Health to disenroll me.

Denver Health expressly reserves the right to terminate a user’s access to any interactive services and/or to any or all other areas of MyChart due to any act that would constitute a violation of these terms and conditions. I understand that Denver Health reserves the right to cancel my use of this service at any time. I will be notified if Denver Health cancels my MyChart medical record access service.

Reasons for termination of access/breach of terms and conditions may include but are not limited to:

- Unauthorized access (e.g., signing on as or pretending to be another person);
- Transmitting messages or material that infringes on or violates the intellectual property rights of others or the privacy or publicity rights of others;
- Transmitting material that Denver Health considers to be unlawful, obscene, defamatory, predatory of minors, threatening, harassing, abusive, slanderous, or hateful to any person (including Denver Health personnel) or entity;
- Using MyChart in any way that is intended to or is reasonably likely to cause harm to the patient or others (including Denver Health personnel);
- Unlawfully collecting information about others, (e.g. telephone numbers, email addresses);
- Intentionally causing cybersecurity threats (e.g. distributing viruses or harmful computer code).

Accuracy and Integrity of Information
Although Denver Health attempts to ensure the integrity of MyChart, it makes no guarantees whatsoever as to the correctness or accuracy of the information in MyChart. It is possible that MyChart could include typographical errors, inaccuracies, or other errors. In the event that an inaccuracy arises, please inform Denver Health so that it can be corrected. Information contained on MyChart may be changed or updated without notice.

Technical Assistance

Down Time
I understand that MyChart may not always be available to me due to system upgrades, maintenance, or downtime. Access is provided on an “as is available” basis and I am not guaranteed access at any particular time.

This Agreement shall be governed and construed in accordance with the laws of the State of Colorado. Venue for any legal action or proceeding arising hereunder shall lie in the District Court in and for the City and County of Denver, Colorado.


You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Denver Health have control over the contents of these services, and neither Epic nor Denver Health accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.
